This isn’t the first time Sam has decided correcting Dean isn’t worth pursuing at this moment.  The first time they have a breather in which Sam could say something to Dean about any of the stuff that’s happening, he chooses to focus on what he thinks maybe the Mark is doing to Dean, because he’s worried about Dean.  Correcting Dean about a thing Dean is going to cling to whether Sam clarifies it or not (because let’s face it, we all know, Sam included, that if Dean has made his mind up about something, there’s no changing it).  I think Sam sees that in the aftermath of Jodi and Alex, he will have time and opportunity to address exactly one issue related to the events of this case, and he chooses Dean.  Dean is the one in danger from some Mark they don’t fully understand yet.  Dean is the one in peril and acting not like himself.  Sam is afraid of losing him. 

In order to choose to bring up the “wouldn’t save me” thing, Sam would have to decide to make himself feel better by getting himself heard over trying to make sure Dean is okay, and that’s just not Sam.  Aside from the points above, Sam is already doing more than he has in the recent few years to safeguard himself and make boundaries, and that shit is exhausting.  I think he thinks he’s doing enough, and doing any more will just be unproductive on top of shifting energy to himself when Dean is the one who is really in trouble.

(because neither of these boys actually considers emotional or psychological injury to be a real thing, all of their collective evidence to the contrary notwithstanding.)
